The skin has lots of small sebaceous glands in the hair follicles. These glands secrete an oily substance called sebum. Sebum is excreted out on the surface of the skin through small pores, keeping the skin healthy, smooth and supple. Due to hormonal changes during puberty and premenstruation there is an excess output of the sebum. At the same time the dead cells lining the opening of the follicles are not shed properly and clog up the follicles. These two combined factors cause a build up of sebum in the follicles. This causes blackheads and whiteheads. For some people, their acne does not progress beyond this stage. However, in some people, the build up of oil in the follicles creates an ideal environment for a bacteria called P.acne’s to grow. These bacteria normally live harmlessly on the skin but when this ideal environment is created, they grow. They feed on the sebum and produce substances that causes a response from the body’s immune system. This inflames the skin and creates the redness associated with spots. In more inflammatory acne cysts develop beneath the skin’s surface. These cysts can rupture spreading the infection into nearby tissue and this can result in scarring.
Everyone who has acne would have tried out various means to get rid of the problem.
Advice from friends and advertisements promising fast cures often does not help. Over the counter medications usually the first to be tried out. These include topical ointments like Benzol Peroxide, Salicylic acid, Glycolic acid, Sulfur, Azelac acid Retinoids. They act by killing the bacteria and by unblocking the pores. However, they have limited effect, and are only good for controlling the spread of acne.
Prescription products such as Oral Antibiotics – minocycline, doxycycline, tetracycline, erythromycin, azithromycin as well as topical Antibiotics like erythromycin gel, and Synthetic Vit. A derivatives like retinoic acid cream, are more effective, but are expensive and can have significant side effects.
Each one of the conventional solutions has its advantages & disadvantages. Some give good results but have severe side effects, others are well tolerated but with poor results.
